You have done such a great job planning and designing useful evaluations for Employer A that another organization (Employer C, pg. 36) wants you to consult with them to provide a program planning and evaluation proposal for their workplace wellness program. The Program Planning & Evaluation Proposal Presentation should provide the following components: an overview of employer and the workplace wellness program, stakeholder identification, logic model, Evaluation Plan and Design, cost-effectiveness OR cost-benefit analysis plan, and recommendations for the workplace wellness program. Format your presentation utilizing PowerPoint. Use no more than 15 slides. The slides themselves should only contain short statements, bullet points, graphs, etc. Add explanatory text that will be spoken out loud by the presenter in the “Notes” box that is below the slide window.
Your proposal should include the following information:
- Overview of Employer and Workplace Wellness Program (need assessment – why do they need a wellness program?)
- Stakeholder Identification – What stakeholders should be included in the workplace wellness program planning process? Provide rationale.
- Logic Model – Including resources, activities, outputs, short-term outcomes, intermediate outcomes, and long term outcomes (program impact or problem solution).
- Evaluation Plan and Design – What performance monitoring measures will you use (minimum 2)? Provide justification and examples. What data collection methods will you use (minimum 2). Provide justification and examples.
- Cost-Effectiveness OR Cost-Benefit Analysis Plan – Describe which analysis you will complete and how you plan to collect the needed information.
- Provide at least 5 recommendations for the Workplace Wellness Program. Provide rationale.
