The Assessment Paper is the common assignment for all sections of the On-Campus Generalist Practice with Individuals Across the Life Span. This paper is designed to help you think critically about your work with a client through your critical analysis of case from your field placement or as an alternative assignment, an audiotaped assessment interview .You must integrate the relevant required readings and at least 3 outside sources to support your analysis of the assessment interview. You will be graded on your ability to demonstrate 1) your ability to write up an assessment based on what you learned from the interview 2) your critique of the multi-level information explored within the assessment interview 3) your critique of the way in which the information was obtained and explored 4) your critique of the case formulation and intervention planning process Instructions for Completing the Common Assignment Complete the following questions about the assessment of your case from your field placement or of the audiotaped interview if you are doing the alternative assignment. Please note, if you are doing the alternative you will need to review all or parts of the tape several times in order to fully and accurately answer these questions. Part I. Write up of the Assessment 1) Describe what you learned about the presenting problem. 2) Describe what you learned about the client on the individual/person level (key demographics and any individual risk and protective factors 3) Describe what you learned about the client’s family (structure, roles/rules dynamics) and the family level risk and protective factors. Attach a genogram. 4) Describe what you learned about the client on the environmental (socio-cultural) level. Attach an ecomap. 5) Based on what you learned about the presenting problem and the information on each of the above levels, write a case formulation 6) Describe an intervention plan that would be responsive to this case formulation Part II. Critique of the Information Gathered on Each Level 1) Discuss what information was gathered on each level (individual, family, environment and the extent to which it assisted the social worker in understanding the client’s situation This discussion should be informed by the relevant published literature that you found about this client population and client situation including what you have learned about the relevant individual, family and environmental risk and protective factors for clients in this developmental stage. Please be sure to discuss any further information you would recommend be explored based on what you learned from the literature. 2) Discuss how the information was gathered and explored including the interviewing skills utilized. Discuss how effective they were based on what you have learned about effective interviewing and client engagement for the client’s’ developmental level and any other factors that could impact the engagement process. Be sure to ground your critique in the required and additional relevant literature. Part III – Discussion of Case Formulation and Intervention Plan 1) Discuss any literature that supports the case formulation and intervention plan 2) Discuss the extent to which the client was involved in the case formulation process and whether the level of involvement was appropriate given the client’s age and developmental abilities. 3) Discuss how well the intervention goals were identified and the extent to which they were measurable, attainable, realistic, and timely (SMART goals). 4) Discuss if there are any changes that you would make to the way in which the case formulation and intervention plan was discussed with the client 5) Briefly discuss how this intervention plan could be evaluated based on what you have learned about evaluating process and outcomes.
