I need a paragraph response to the post below:
The National Quality Forum (NQF) defines Never Events as errors in medical care that are of concern to both the public and health care professionals and providers, clearly identifiable and measurable (and thus feasible to include in a reporting system), and of a nature such that the risk of occurrence is significantly influenced by the policies and procedures of the health care organization. Despite the widespread usage of the term “never events,” the NQF refers to these events as “serious reportable events” whereas the Centers for Medicare and Medicaid Services (CMS) have deemed as “non-reimbursable serious hospital-acquired conditions.” Some examples of these events consist of wrong-site surgery, mismatched blood transfusion; major medication error; severe pressure ulcer acquired in the hospital; and preventable post-operative death (Lembitz & Clarke 2009). It’s important that health care organizations are providing quality assurance to meet the appropriate standards to promote efficient health care and continuously improve the outcome of health care services.
Nurses play a key role in quality improvement by collaborating with other health care professionals, implementing evidence-based practices, following orders protocols and policies, including patient and family involvement, communication and bedside shift reporting, and providing culturally competent care. The “always events” represents a positive affirming behavior that can motivate nurses to improve patient safety and promote better outcomes. Some basic examples of “always events” include patient identification by more than one source, surgical time-out, Mandatory “read-backs” of verbal orders for high-alert medications, medication error reduction strategies (Lembitz & Clarke 2009).
Nurse-specific challenges in influencing change in quality improvement can be the accessibility and availability of resources, lack of collaboration amongst team members, work environment, workforce shortages, misuse or underuse of medical services, and lack of medical supplies and equipment. It’s essential to improve health care systems to enable nurses to provide the best care and ensure that patients will benefit from safe, quality care.
