Given a case study, outline an appropriate plan of care that identifies the anticipated nursing interventions that should be implemented to address the patient’s priority needs. Consider timeliness, accuracy, safety, best practice and relevance as interventions are developed.

Step 1. Introduction (2-3 paragraphs)
After reviewing the GI Case Study, describe your initial thoughts about the patient, his current status and the plan of treatment.

Step 2. Complete the Evaluation of Skill Performance Chart (3-5 pages)
Complete the chart below.

  • Begin the chart with the recommended nursing interventions in order of priority
  • Provide rationale* for each intervention
  • Explain how each intervention aligns with best practice
  • Determine the expected patient response to the intervention
  • Step 3. Conclusion (4-5 paragraphs)
    You have completed the Plan of Care and are thinking about discharge planning.

    Based on your observation and reflections, identify the overall goal of the plan of care. What teaching would be involved and why? How will you include the patient’s family? What kind of follow-up is required? Who are their supports? Are referrals to external agencies needed?
