Case management, as defined by Woodside and McClam (2019), involves “the activities of planning and coordinating, and, in some cases, delivering treatment strategies” (p. 279) and is an important service delivery strategy. After reading the required chapters from the Woodside and McClam text, the articles Case Management at the Intersection of Social Work and Health Care and Strengths-Based Case Management: Individuals’ Perspectives on Strengths and the Case Manager Relationship, and watching the video Case Management in Health Care, discuss the basic features associated with case management and explain the case manager’s role. Then, reflecting on the case management section from Chapter 1 of the course text, discuss the three factors that created the need for case management.

In the field of human services, case management is a vital component. With the help of case management duties, you may provide many services in a more organized and efficient manner. Case managers are in charge of gathering data, conducting assessments, and monitoring services. They also coordinate services from other organizations, serve as advocates, and oversee the distribution of resources and the assurance of their quality. When it comes to their patients, a care manager has a number of objectives in mind. Listening, showing empathy, and allowing oneself to be vulnerable to your clients are the most effective ways to help them. In order to build trust and gather data, the care manager must correctly decipher it so that the client may help themselves in their current situation. Case managers should always demonstrate compassion to their clients since they are seeking someone to talk to and who will be able to accept, respect, and care without casting judgment on their situations. The management’s treatment programs should be the most effective. Patients and their families come first, and the case manager works closely with them to make sure they have access to resources, keep track of how they’re using them, and help them become more self-sufficient. Strength-based management improves a client’s chances of success, even in the most hopeless scenarios. Individualized treatment plans are developed with the client’s strengths and preferences in mind. The rapid rise in healthcare expenses has prompted numerous modifications in managed care during the past decade. Manager care had to make major cuts to conserve money as a result of these modifications. Smaller community-based facilities can provide more precise and controlled services than huge state-run institutions. This was the first step in helping these organizations better serve their clients. It’s also worth noting that historically, public funding was allocated based on a client’s specific needs rather than a more holistic approach. As a result, services for clients with multiple issues were often overly complicated or fragmented, making it difficult to coordinate efforts across different agencies and jurisdictions. Third, the current state of human services employment necessitates a change. Worker multitasking can generate issues with time management, efficient service delivery, and quality assurance, as well as the coordination of services from other organizations and specialists (McClam, 2019). People who can’t afford medical insurance or medicines are always affected by government health care reforms, and this is especially true for those who are poor and unable to afford basic necessities like food and shelter.

According to (McClam, 2019), case managers duties are to gather information, give evaluations, observe services and also to arrange service from other institutions, provide advocacy services, take responsibility for distributing resources and supplying quality assurance. Another duty of the case manager is to gather information for the client and provide them with other institutions that can help them turn things around on a positive note. Looking at the video “Case Management in Health Care,” it talked about how the client should be the case worker’s number one priority. The case worker needs to focus heavily on setting proper goals for their client to achieve, which also includes settings goals for when the sessions ends.
When it comes to factors that created the need for case management, there are definitely some that can be given. The first is the shift from large state-operated institutions to smaller, community-based facilities. (Woodside & McMcam, 2019). The second is categorical public funding of the past, which made the services very hard to handle and made things complicating. The third is nature of human service work. This simply means that agencies can get additional help with clients from case workers, which ultimately makes things easier for everyone.
