Case segment 1 (Medical Indications & Patient Preferences): Case segment 1 should be no longer than 2-3 pages, typed, double-spaced. This segment should include the first two assessment sections. Student analysis writing should demonstrate assessment skills (describing what is observed in the case) and higher-level skills. These higher skills include, for example:
- For medical indications, synthesis of assessment data to determine appropriate goals for care for a patient under these circumstances; evaluation of the appropriateness of a medical intervention given its (lack of) correspondence with the most appropriate goals of care; evaluation of the appropriateness of a medical intervention given the expected ratio of benefit to burden or risk.
- For patient preferences, evaluation of an individual’s capacity to give consent to treatment or to refuse recommended treatment; synthesis of assessment data to determine the appropriate surrogacy standard; application of the appropriate surrogacy standard given the facts of the case. When possible, students should explain their judgments with reference to accepted criteria, such as Grisse and Applebaum’s functional criteria for assessing capacity.
