Submit a performance evaluation instrument based on a job description for use in evaluating staff performance in a child care center (birth to five) and describe how a director would us it in an evaluation process. You must do research to locate a performance evaluation tool. You can speak to a center director and ask for a copy of their tool, use one from one of your text books, or conduct an online search. You must submit the actual tool and provide a written narrative answering the below questions thoroughly. Submit tool by uploading as a scanned document, a pdf or cut and paste to a word document.

Include and identify the instrument by title, author, and date of publication or copyright. The description be written in essay format, should be a minimum of 1 page long and should include:

  • Which staff will be evaluated (lead teachers, teaching assistants, cooks, etc.)
  • How frequently the evaluation will occur (yearly, bi yearly, etc.)
  • The purpose for using this particular instrument; what is to be determined (merit raises, training needs, etc.)
  • Who will conduct the evaluation (director, assistant director )
  • How will the information that is gained be used? (improve performance, coordinate trainings, promotions, etc.)
