Steps to Design a Concept Map

  1. The first step is to choose one of your assigned clients.
  2. Once a client is chosen, collect information about the client’s health problems. 

Utilize the medical record, interview the client and complete a physical assessment.

  • In the center box of the concept map, write the reason for the client needing health care (i.e. medical diagnosis, medical/surgical history, surgical procedure).  
  • In the boxes surrounding the center box, identify and list the four most important problems for this client in priority order.  These boxes are also to include the evidence that you found from your client (be specific) that supports the identified problems.  The evidence is to include; assessment data, laboratory values, and results of diagnostic tests.  If supporting evidence is relevant to more than one problem it can be written in more than one area.
  • List the medications and interventions that are relevant to the problem in the attached box. 
  • Evaluate client’s response to medication and interventions.
  • Avoid using Nursing Diagnoses
  • Use Bullet format in each section
  • Concept Maps are to be submitted with Weekly Clinical Assignment for the client.
