I need 4 soap notes about 4 different patients. 1st is male 68 yrs old, 2nd male 46 yrs old, 3rd 81 year old female and 4th 59 yr old male. I will attach 2 example Soap Notes to see how it should be written. Please follow the examples for all 4 patient soap notes. I will also attach a blink template to use has an extra guide. Everything is straight forward. I don’t need any reference page, therefore can be creative with everything. Again , PLEASE FOLLOW THE EXCAT FORMAT OF THE EXAMPLES I ATTACHED.
