The course, certificate or license report should be a minimum 15 pages, excluding the title page, acknowledgements, and reference page. The report
should be double-spaced; using the Arial 12-point font (or equivalent), with one-inch margins, and page
numbers in the right-side footer. APA-style formatting is only required for the in-text citations and the

statements. Since the course, certificate, or license happened in the past, most of your report will be written in
the past tense.
Each paragraph should have a 5-space indentation. Do not quadruple space at the end of a paragraph. There
should only be a double space between the end of a paragraph and the next paragraph. Be sure that your
paragraphs are set for 0-point spacing.
The report should contain the following components:

  • Title Page: This should include your name, your Net ID, and the term in which you enrolled in HCA693. Note: the term in which you enrolled should be on the title page, not the term in which you are submitting the report. Your term of enrollment is Fall 20. The title page does not contribute to the required minimum pages
  • Overview of Course, Certificate, or License: In this 1-page section you should describe the course you
  • completed and the college, university or organization that offered the course or you should describe the

    type of certificate or license you obtained and the organization that granted it. After reading this section,

    the reader should have a fairly clear view of the course you completed or the type of certificate or

    license you received, the organization that offered it, and what new employment opportunities

    completing the course or having the certificate or license opened for you. Do not copy/paste any

    material from the organization’s website or promotional materials. If you plagiarize any part of this

    section, you will not earn a passing grade in HCA 693. This section may be longer than 1 page, but the

    minimum required page length of the other sections remains the same and the total length of the report

    cannot exceed 21 pages.

  • Skill Sets and Knowledge Base: This section should contain a broad breakdown of the skill sets and the knowledge base that you acquired and how they were acquired. If you obtained certificate or license, this section should also discuss any kind of training that you received in preparation for the certificate or license exam. This section of the report must be a minimum of 14 pages. This section may be longer than 14 pages, but the minimum required page length of the other sections remains the same and the total length of the report cannot exceed 21 pages.
  • MS-HCA program: What new skills did you develop or strengthen in the MS-HCA program that prepared you for the course, certificate or license and/or have increased your likelihood for employment in the health care field? This section of the report should be a minimum of 1 page. This section may be longer than 1 page, but the minimum required page length of the other sections remains the same and the total length of the report cannot exceed 21 pages.
  • • Reference Page: This section requires in-text references and citations to document sources consulted in
    preparing the report. This consists of a list of all the books, articles, manuals, internet sites etc. cited in
    the report. There should be a minimum of 10 references. You must use APA-style formatting for the

    to the required minimum pages.

  • Appendix: The Appendix should contain the documentation of the completion of the course (including the grade earned), certificate or license. If you participated in a training program in order to prepare for the license, documentation of the training should also be provided here. The Appendix may be numbered but it does not contribute to the required minimum pages.
