A- How do I got affected by COVID-19?
For A section , you can use the following reasons.
1- I lost my job ( I used to do In Home Suppor tService to my mother and she passed away on July,31,2020)
2- I have a bad health condition ( I will need a right knee replacement surgery soon + I have a heart problems and I am following up with my Cardiologist )
B- Why do I need assistances?
For B section , you can use the following reasons.
1- To be able to help my husband so we can pay our rent and utilities bills.
2- To be able to pay medicine copay.
3- To be able to buy daily life supplies.
