- Collect outcomes data on the concept to provide evidence for quality improvements in clinical setting. Data must be measurable.
- In relation to the chosen concept, Identify practice discrepancies between identified standards and practice that may adversely impact patient outcomes.
- What are the contributing factors for the discrepancies, think about as many possible causes as you can determine for your chosen concept reflecting on your past clinical experiences and literature.
- You will come up with 3 root causes for the concept.
- Action Plan. Discuss improvement methods, based on data from the outcomes of care processes to continuously improve the concept in the setting chosen. Based on evidence-based practices and root causes, develop an action plan of nursing strategies/interventions to support preventing the discrepancies from future occurrence. In-depth action plan should include all three components: actions/ interventions, responsible person(s), and timeline. These action plan should base on your literature search. This should not be what the organization is already doing.
- How will your action plan impact care quality, patient satisfaction, and cost of services within this facility/organization?
- Dissemination of Plan. Describe how you will disseminate your action plan within the facility/organization.
- Support quality improvements, policies, quality initiatives and data with quality references.
- In order to complete this assignment, you will need to incorporate theory and research and prioritize data from at least 5 sources. This should include textbooks, at least 3 nursing journals, and credible web sources (CMS, JC MDH).
