I selected the accident #121071.015. According to OSHA, falls are one of the most common causes of death at the workplace. This accident happened on 2019. An employee was working on the second floor of a four-story building. The employee fell through a floor hole and died of unspecified injuries. The best way to avoid an accident like the previous one from happening again would be to use guardrails ( engineering controls) around floor holes. Engineering controls place a barrier between the hazard and the employee. If a place is too narrow, the floor hole could be covered with a plank or plywood. The protective cover should cover the entire opening and should be securely fastened. Also, protective covers should be able to withstand any load that might be put on it. Protective covers should also be labeled. Floor holes should never be ignored, since they can lead to severe injuries or even death.
