Connect for Health has several programs that they provide to help patients achieve a promising state of health. After working with several clients and listening to their stories, the problem that is constant is homelessness. Connect for Health has resources to help individuals seek section 8 and additional low-income housing, but more needs to be done to support homelessness prevention.
Current efforts to expand the reach and scale of this intervention
If I was tasked to expand on the homelessness prevention program, I would first partnership with schools, churches, safe houses, and shelters to gain access to their services to place homeless individuals to achieve stability for a short time until a more long-term place of residency can be provided. In addition, the effectiveness of the efforts and scale of this program would not only find shelter but create career pathways that will help support individuals as they leave temporary churches, and shelters into permanent housing with meaningful and maintainable job training and employment, access to comprehensive healthcare, safety, stability and community involvement that can aid in alleviating the impact of homelessness, stop criminal involvement of homeless individuals and continue to bring awareness to homelessness in America to prevent homelessness (“Solutions,” n.d.).
Evaluation questions and feasibility to expand my program
With the implementation of my homelessness prevention program, I would ask individuals what causes them to become homeless, and how can my program be a service to them to prevent homelessness in the future? In addition, I would want to know if the partnership resources I have provided for temporary stay were sufficient? and if they would recommend my program to other homeless individuals and families. To further expand and sustain the program, I would take notes on how the program is progressing and create a roadmap for accomplishing long-term goals (“Module 5: Planning for sustainability,” 2021). This would be accomplished by planning and implementing strategies that will continue to allow me to partnership and build sustainable relationships that will improve health, prevent homelessness, and help the program thrive. Moreover, through interviewing, surveying, networking, and assessments, will allow me to understand if expanding my homelessness prevention program would be beneficial or not.
