Goals/objective
The objective of the ages and stages questionnaire is to detect any delays and improve the
child’s outcome. 24-month-old Joshua Lambardo seems to good communication, gross motor,
fine motor, problem-solving, and personal social skills similar to any normal toddler. Therefore,
this questionnaire was essential in detecting any skill that seemed out of place or behavior that
was not normal. Each skill has a cutoff point which assists in understanding the child’s behavior
(Brookes, 2014). An additional objective for the screen is to encourage parent involvement in the
child’s life and education.
Action Plan
The action plan is to ensure that Joshua maintains the same level of care from his parents.
This action plan is essential since, through screening, Joshua has shown that he is growing up
like a normal toddler and showing normal symptoms. Therefore, with nothing to worry about,
the parents have to maintain the same level of care for their son.
Intervention activities (Describe the activities that would support the child)
The child hears well, and he talks like other toddlers. I understand most of the words he
says, walks, and runs normally. The family has no history of hearing impairment thus has no
vision problems, and the past medical problem he faced was a common cold. Therefore, I have
no concerns about his behavior or any other concerns. Thus, there is no other action to take with
Joshua Lambardo (Brookes, 2014).
