    • Scenario #1 – Female Genitalia:Ms. G., a 23-year-old Hispanic female, came to the clinic for an annual Pap Smear and complains of having a thick, cheesy, white vaginal discharge with itching that has been present for one week. She denies burning during urination. Ms. G. is sexually active in a monogamous relationship with her boyfriend of three years. She states that she and her boyfriend have no other sexual partners. She denies fever, chills, abdominal pain, or pelvic pain. Her last menstrual period was three weeks ago. She does not have any children and is currently attending a local community college and does not have any health insurance. She states “I don’t know how this could happen to me; I am only seeing my boyfriend”. She indicates she is satisfied sexually and she uses oral contraceptives.
      1. Based on her presenting signs, what other questions would you elicit?
      2. How would you demonstrate knowledge of cultural sensitivity/humility in approach to this patient?
      3. What would you assess?

      Scenario #2 – Male Genitalia and Hernias:You are working at a health fair. Mr. C., 48 year-old Haitian male, seeks advice from you because he has been “unable to maintain an erection and has bleeding from his rectum, and has observed a small amount of bright red blood in his stool for the last three days, and feels a pressure when he has to go to the bathroom”. He complains that his bowel movement is sometimes hard. Mr. C is a Taxi Driver who has no insurance through his work. He denies back pain, blood in urine, or recent trauma. He has a history of high blood pressure. He states that he cannot remember the last time he had a physical or went to a doctor for illness. He does not perform testicular self-examination (TSE). He has smoked two packs of cigarettes daily for the last 10 years. He admits drinking 3 to 4 beers occasionally. He is married with three children and is not satisfied sexually in the marriage.
