A logic model is a graphic depiction (road map) that presents the shared relationships among the resources, activities, outputs, outcomes, and impact for a program/project.
To complete the assignment:
- After reading Chapter 12, create a Logic Model of your Program Design project.
- The logic model should include inputs, outputs, and outcomes. How you lay these out will depend on what your project aims are and the intended audience for your logic model.
- There are 2 fillable Word documents provided as templates below. You can use either of these, the sample layout from the text, create your own, or find another example.
- Before you begin to develop your logic model, be sure you know the answers to the following questions:– What is the problem or situation that the logic model addresses?
– Who will use the logic model and what will they use it for?
– What are the boundaries of the logic model?
– What have others done (or are doing) for similar problems or situations?
