A factory contains an area where robots do all the work. There is a fence around the area. Human workers are not supposed to enter while the robots are working. When anyone opens the gate in the fence, it automatically cuts off power to the robots. A worker, however, jumped over the fence to repair a malfunctioning robot. Another robot, bringing parts to the malfunctioning robot, accidentally pinned the worker against a machine and crushed him to death.

You are part of a compliance team hired to investigate. Consider several factors relevant in safety-critical systems. What was done right? What was done wrong? What other important information do you need to know? What degree of blame do you assign to the software company that designed the robot system, the factory, the dead worker? What changes, if any, should be made?
