evaluation session you might complete as a student clinician. It will
contain a case history section, a formal evaluation section, an informal
evaluation section, summary and recommendations, and diagnosis and
prognosis.Review the Case History power-point–a short tutorial on taking a case
history. Then review “your client’s” case history that I provided. Write
the case history section of your diagnostic report—I have provided
several report samples, so these should help guide your writing style.This is not gonna be in an essay form so please review the samples and the step by step instructions which will all be attached.
