Use a narrative format.  Make sure that each of the 13 areas in the Head-to-Toe Performance Checklist ae addressed in your documentation.  Use appropriate medical terminology and phrasing.  Must select 7 different areas of the 13 *BOLDED areas from the Performance Checklist and document abnormal finding/s (cannot utilize the same area more than once).  When an abnormal area is identified must describe the abnormality in parenthesis. Example:
