Other history associated with this 61 year old man includes Ischemic Cardiomyopathy, End stage heart failure, Diabetes Mellitus Type 2, Peripheral Vascular Disease. He is allergic to Allopurinol, Tramadol, Enalpeil, Compazine.
Diagnosis: CHF Exacerbation
History of present illness
A 61 year old male patient with history of ischemic cardiomyopathy, end stage CHF with ejection fraction of 10 to 15%, status post biventricular ICD on 07/2020. history of coronary artery disease, status post CABG2009, HYPERTHENSION, hyperlipidemia, Periperal Vascular Disease, Diabetes mellitus type 2, obstructive sleep apnea, chronic kidney disease stage iv, liver cirrhosis, benign prostatic hyperplasia, and recurrent left pleural effusions status post multiple thoracentesis, who was admitted with chest pain, CHF exacerbation to acute facility to continue treatment, with a plan to continue treatment with a plan to go home on Milrinone drip. The patient has a foot ulcer over the medial aspect of the right foot
