Choose a disease or health condition on which you will base your final paper (proposal).

Review the final assignment. This will help with your decision. Determine a local jurisdiction on which you would like to concentrate (city, township, county, or state). Examine the epidemiology of the chosen disease, reviewing its history and trend, comparing the national trend to the chosen local jurisdiction. Describe the local jurisdiction (population and demographics). This will be the introduction to your final assignment.

Part 2

Write 1-2 pages( 400–600) words that respond to the following questions with your thoughts, ideas, and comments. This will be the foundation for future discussions by your classmates. Be substantive and clear, and use examples to reinforce your ideas. Utilizing tables may be helpful for presentation.

Using CDC Wonder, choose Diabetes, In Atlanta- Georgia, USA a time period (years). Review the data covering a 5–10-year period. Answer the following questions:

What are the morbidity and mortality rates for the health condition or disease?
Choose 1 year, and review the data by age, ethnicity, and gender. Do you observe any disparities within these groups?
What pattern or trend have you observed over the 5–10-year period?
What are the risk factors for the disease or health condition?
Does this information surprise you? If so, why?
How can these data be used to inform policy and prevention and intervention programs?

Part 3 Continue to use Diabetes in the Atlanta, Georgia area USA

A component of your final project is to complete a needs assessment of your target population pertaining to the disease or health issue for which you are proposing the need for an intervention or prevention program. A component of the needs assessment is to describe the disease or health issue to demonstrate the need. Using the epidemiologic triad of people, time, and place, provide a descriptive analysis of the health issue and the impacted population.

For data on your chosen disease or health issue, review the following data sources:

CDC Wonder
Community Commons (You have to register for this one, but it is free.)
U.S. Census Bureau (This is good for demographic data.)
County Health Rankings

Describe the disease in terms of the following:

People: Demographics of those impacted (age, race/ethnicity, gender)
Time: The time period of the data you are reviewing (5–10 years, 1 year, etc.)
Place: Built environment and other environmental factors

Represent the data in graphs, charts, and tables as relevant, accompanied by a brief explanation.

Describe the morbidity, mortality, incidence, and prevalence rates of the data. Describe differences in terms of age, race/ethnicity, and gender. Which age group, race, or gender seems to be most impacted by the morbidity and mortality data? What were the morbidity and mortality trends for the time period observed (5 or 10 years depending on the data)?

This part should be 2–3 pages in length and APA-formatted with in-text citations and references, and be able to stand alone as a paper.

The concept behind the 3-4-50 model is that there are three unhealthy behaviors that influence four chronic diseases, which leads to 50% of all deaths. The percentage of deaths can vary by community or place. For example, in some communities, this can be 3-4-63 or 3-4-45.

Can you find the mortality of these 4 diseases in your community?

Physical activity, or lack thereof, is 1 of the 3 behaviors that impacts the 4 diseases. What is the walkability index for your neighborhood (area)?
- Does this support the data as they relate to health conditions that are impacted by physical activity?
From the data and the information you gathered, what recommendations might you have for your community?

Using the information from the previous parts , complete and submit Part 1 of the first draft of the Key Assignment. This should be 3-4 pages- excluding title and reference pages

You are the Program Manager, and you are trying to decide on the best course of action to decrease incidence and prevalence of the disease chosen in Week 1. You must decide from the data which population is most at-risk and decide on the most appropriate intervention to which you should allocate funds. To do this, you need to see the big picture. In this assignment, you will write a proposal to your Chief Executive Officer outlining the following:

History of the disease (show trends and data)
Needs assessment for your population:
- Demographics
- Social factors that may increase risk (poverty, health insurance, race/ethnicity, etc.)
Morbidity and mortality data
Incidence and prevalence data
Suggested intervention program with cost analysis
Justification and explanation for why this is the most appropriate intervention for the target population

Part 6

This Takes all of the previous work and combines it into a final Key Assignment. This will add an Additional 2 pages for a final draft of 5-7 pages excluding title and reference pages

Take part 5 and add the following- For your final draft, include the following information, focusing on the evaluation of the program once it is implemented:

What type of data would be collected, and when?
What tools or measures would be used to collect the data (e.g., surveys, questionnaires, pre and post assessments, town hall meetings. etc.)
What would be used for evaluation (rates, mean, etc.)?

Cite your sources using APA format, and remember to include a citation for the article that you are describing
