Using the National Alliance on Mental Illness (attached) Types of mental health professionals and Summary of DSM-IV Diagnostic Criteria (attached) for conditions commonly seen in primary practice care web links and your UNIT 1 WIKI as your guide, identify different health care professionals within your community that you can potentially refer your patients to.
Once you have reviewed steps 1-4, using any software that can produce flowcharts (e.g., Microsoft Power Point, Microsoft Publisher, Lucid chart), create a plausible referral network/process for your practice. See example attached as (student sample) or google different examples on the Internet. You may choose to organize your chart in any way you want to best represent your practice settings.
