An intervention or planned change activity typically moves through several sequential phases, with each phase building on the previous ones. The uniqueness of each phase is that the social worker is focusing on specific actions, information, decisions, or outcomes. If a social worker is to guide the change process, he or she must become an expert regarding the tasks that must be accomplished in each phase.
