Choose a case. Make sure that the case has enough clinical complexity as demonstrated by an individual or family having difficulty in multiple areas of life. Present your case from a purely clinical perspective. (Remember, later you will focus on community and organizational issues so make sure the case has enough community and organizational complexity to provide enough material necessary for this assignment but do not discuss that here).

You will discuss the treatment of this client or these clients according to the chosen models. This case may be one of your own or one provided by the instructor. Either way, a 2-page synopsis of the case will be necessary.

Length: Your paper should be 20–24 pages in length with 10–12 pages for Clinical (Micro) material (Critical Elements I, 1-8) and 10–12 pages for Community/Organization (Macro) material (Critical Elements II, 1-6). Use the number of points assigned to each question to guide you on the length, depth and breadth of your answer with a minimum of 10 pages for each half as instructed.

References and APA (6 points for Clinical; 6 points for Community): Use a minimum of three (3) different references in the required readings from the clinical (Macro) module and a minimum of three (3) different references in the required readings from community-focused (Macro) module. In addition, you must use a minimum of four (4) additional outside resources for any part of your paper. Use correct APA style (title page, running head, references in narrative, reference page). No abstract required.

The following CRITICAL ELEMENTS must be addressed:

I. CLINICAL CASE (150 points):

  1. Present Your Case (12 points): Make sure that the case has enough clinical complexity as demonstrated by an individual or family having difficulty in multiple areas of life. For this paper, it is not necessary to have actually done these interventions, just imagination is sufficient. This case may be one of your own or one provided by the instructor. Either way, a 2-page synopsis of the case will be necessary.
  2. Clinical Theoretical Model (18 points): This paper requires that you choose one theoretical model/framework to guide your approach to one specific complex clinical case. Select one of the complex approaches to clinical work covered this semester, such as AMBIT, SBRIT, Boyd-Franklin’s Multisystems Approach, DBT, CBT, Attachment-Focused Family Therapy, EMDR, etc. Clearly state the model you chose and how it fits this case. Use your imagination to demonstrate that you know how to apply a specific approach to a complex case. Choose a model that “fits” the case then support your choice to use the theoretical model you chose with evidence-based references.
  3. Assessment (24 Points): Based on the model chosen and covering the points spelled out below, write a succinct assessment. Do not restate the “facts” or the material in the synopsis. Organize your assessment in a manner consistent with the theoretical orientation chosen. Include:
    1. The most relevant history, including any relevant “biological” and/or “transgenerational” information.
    2. A brief statement of the presenting problem(s). c. In a separate section, be sure to include your “impressions” in professional (theoretical) language. Include in this section the developmental themes you detect from an individual life cycle or family life cycle perspective.
    3. Client(s) strengths and vulnerabilities.
    4. Describe pertinent psychosocial factors.
  4. Engagement Issues (12 points): What are the engagement issues? Are there gender, sexual orientation, class, race, cultural, and/or generational concerns to navigate? Are there resistances or obstacles? How do you work through these challenges? Note your attention to these questions. Clearly consider each aspect.
  5. Treatment Plan (30 points): Describe the treatment plan you are proposing. (Stay consistent with the theoretical model you chose.)
    1. Which treatment modalities did you choose (individual, joint, family, group, case management) and why did you chose these in this case?
    2. What short-term goals are you setting? (Be specific).
    3. What long-term goals are you setting? (If short term-therapy is used, speculate on what longer term issues might be present in a year or more).
    4. What specific interventions will you use to meet the goals you have set? (A minimum of three different interventions should be identified and then describe how these would get played out with the client(s). Again, the interventions utilized must be consistent with the theoretical model being utilized.
  6. Therapeutic Relationship (24 points): Discuss transference/Countertransference/Mentalizing/Intersubjectivity/Impasses/Salient Middle Phase Therapeutic Relationship Issues, etc. How does this client relate to you and you to the client? What significant impasses did you hit?
  7. Clinical Ethical Dilemmas (6 points): At least one complex ethical dilemma or potential ethical dilemma should be articulated and discussed.
  8. Transfer/Termination (18 points): Anticipate the transfer/termination process. How much time do you anticipate for this phase? Include a goal review. What themes might be highlighted? Do you anticipate any problems? How do you think the client(s) will handle this phase?

II. COMMUNITY/ORGANIZATION MATERIAL (150 points):

*Note: While the first part of the paper required that you consider the “pure” version of the clinical approach, this second part of the paper requires that you consider the case within the context of the organization and community.

●Organization (6 points): Identify the organization within which this client is being seen and give a brief description of the organization including location, size and mission. Identify the agency as governmental, nonprofit or for-profit.

●Impact of Macro/Community-Level Elements on Client (36 points): Identify other macro/community level elements, structures, and/or systems that impact the client and might show up in clinical treatment. Taking into consideration the clinical model/approach you have chosen to take with this client, how might you as the clinician gain a better understanding of these elements, structures, and/or systems (both positive and negative) how might you:

a.as the social worker in a clinical practice role, integrate the macro/community elements into the therapeutic process (18 points)

b.as the social worker in a community practice role, work on the macro/community level to “to enhance human well-being and help meet the basic human needs of all people (Social Work Code of Ethics Preamble).” (18 points)

●Brief Screening Tools: (24 points): Select a brief-screening tool based on the organization and the organizational lens through which the client is seen. Apply the screening tool to your case. Explain why you chose this screening tool, being sure to address both micro and macro elements that went into your decision.

●Agency’s Funding: (18 points): What is the funding source? How is the client and therapeutic process impacted by this funding source and method of payment? Perhaps the agency funding is impacted by the client. If so, discuss that. What is the process through which the organization receives funding?

●Organizational or Community-Level Ethical Dilemmas (18 points): At least one complex organizational or community-level ethical dilemma or potential ethical dilemma should be articulated and discussed. Consider issues pertaining to management, administration, non-profit boards, and community expectations among others.

●Public Health Issue Impacting the Client (24 points): Identify and assess a public health issue impacting the client. How does this issue impact the client? Are there coping skills you would need to address with the client? Are there ways might you empower the client and the community to address this issue?

●Impact of Behavior Health Disparities (18 points): What behavior health disparities impact (or potentially may impact) this client? What cultural and community issues are to be considered to address these disparities?
