Examining Your Own Modifiable Disease Risks
The health facility for which you work has asked all employees to take a Life Expectancy questionnaire about modifiable health risks and provide written responses. The results are for your own use. The purpose of the questionnaire is not only to raise awareness of your own modifiable health risks, but to reflect on how modifiable health risks can be addressed in patients.
First, complete the Life Expectancy Calculator assessment (http://mym.cdn.laureate-media.com/2dett4d/Walden/HLTH/2115/CH/mm/lifeexpectancy/content/hlth2115_lec.html) and, using the following prompts, reflect on your results, considering how accurate you think the results are and any results that surprised you.
Provide a written response to each of the following:
- Summarize your results from the Life Expectancy Calculator assessment and analyze your initial reactions to those results. (Recommended length: 1–2 paragraphs)
- Describe at least 3 modifiable risk factors that may impact your life expectancy. (Recommended length: 1–2 paragraphs)
- Describe strategies you could use to address these factors and improve your daily health practices. (Recommended length: 1–2 paragraphs)
- Explain which social determinants of health might be contributing to your personal risk factors. (Recommended length: 1–2 paragraphs)
- Formulate one long-term SMART goal and one short-term SMART goal that you think will increase your life expectancy and reduce your risk of contracting preventable diseases. (Recommended length: 1–2 paragraphs)
- Explain how you intend to hold yourself accountable for the realization of those goals and how you will track your progress. (Recommended length: 1–2 paragraphs)
- Explain how evaluating your own modifiable risk factors may help to inform the way you relate to patients.
