Discussion 1: Soraya
1) How has the implementation of information technology and online clinical systems affected patient confidentiality? Comment on ways to maintain the privacy and confidentiality of patient information in light of the use of social media.
The implementation of information technology and online clinical systems has affected patient confidentiality by making it easier for the information to be accessed. The advances in technology in healthcare have been rapid and amazing but it is also the reason for discrepancies. With the increase of electrotronic data there is also an increase in the amount of people who can access the information. Someone halfway across the globe can have access to patient information at any time. Also there is more opportunity for data to be hacked and confidential information to be leaked. Healthcare facilities constantly need to be updating their technology and using software to help prevent data breaches. Seh et al. states, “The frequency of healthcare data breaches, magnitude of exposed records, and financial losses due to breached records are increasing rapidly. Data from the healthcare industry is regarded as being highly valuable. This has become a major lure for the misappropriation and pilferage of healthcare data” (Seh et al, 2020).
According to Spector and Kapel, Nurses have the obligation to establish, communicate, and enforce professional boundaries with patients in the online environment. They must use caution when having online social contact with patients or former patients. Online contact with patients or former patients blurs the distinction between a professional and personal relationship. The fact that a patient may initiate contact with the nurse does not permit the nurse to engage in a personal relationship with the patient. NCSBN (2011a) provides a professional boundaries brochure free of charge”( Spector & Kapel, 2012). In order to maintain privacy and confidentiality of patient information in light of the use of social media I believe that it should simply be avoided completely. As health care professionals we should not post about our patients. According to Spector and Kapel, “Nurses must always be aware of potential consequences of disclosing patient-related information via social media and mindful of employer policies; relevant state and federal laws; and professional standards regarding patient privacy and confidentiality. Patients should expect a nurse to act in their best interests and to respect their dignity” ( Spector & Kapel, 2012). You may disclose information that you felt was appropriate for the world to know and others feel differently. That decision could cost you your career.
2) How can the nurse ensure that the caring, interpersonal relationship with patients is intact despite the advancing technology, and the need to continually interact with information systems?
It is important for nurses to know what their purpose is as a nurse and what their obligations are to their patients. Nurses’ jobs are to provide care, prevent harm, and promote healing. Advancements in technology have made the job both easier and in some as more difficult. For example, having the ability to get vital signs by pushing a button at a computer at the nursing station is amazing. It helps the nurse with time by cutting down on having to go back and forth. On the other hand it removes an opportunity for the nurse to have an extra face to face interaction with their patients. The patient may have needed something and was uncomfortable pressing the call light because they did not want to “be a burden”. This in turn could lead to worse patient outcomes and patient satisfaction.
The need to constantly interact with information systems is also tough on the nurse. The amount of electronic documentation that is needed has increased tremendously. Many times nurses need to stay late at work just to complete documentation. “Some studies have found that although the quality of documentation of care may improve, there is an increase in the time associated with completing computer-related tasks (DesRoches, Donelan, Buerhaus, & Zhonghe, 2008; Kossman & Scheidenhelm, 2008).”(Blais & Hayes, 2016). Nurses are spending a lot more time having to keep up with documentation and that in turn is affecting patient care.

The handling of patient private information has evolved as a result of technological advancement and social media. According to Petersen (2018), technology-induced improvement in quality of life is a constant theme in patient-centered care today. New technologies are a novelty and create new methods of using health data. Despite these benefits, there are many challenges associated with technology and social media, one of which is the issue of privacy and confidentiality, which is why Nassim and Wood (2018) asked the question- is privacy, privacy?
A breach in protected health information adversely affects relationships and even a facility’s reputation and is a potential source for litigation (Nissim and Wood, 2018). Nissim and Wood (2018) discuss innovative ways of tackling the issues of patient privacy breaches. According to Nissim and Wood (2018), technical measures such as data swapping, query logging, and auditing mitigate informational privacy risks.
According to Bender et al. (2017), a simple action of clicking a link leaves a potentially identifiable trail that may compromise the entire system’s security. As a result of social network leaks, Bender et al. (2017) emphasize the privacy by design approach, a world-renowned approach to privacy protection. According to Bender et al. (2017), the privacy by design approach is a proactive measure that predicts and prevents privacy-invasive events. Protecting patient information in this social media age includes moderating comments and content on social media; and posting with caution (Petersen, 2018).
Kornhaber et al. (2016) claim that even though we live in a technologically advancing world, humor facilitates therapeutic bonding and engagement. Humor reduces tension, builds trust, and enhances the development of patient-practitioner therapeutic interpersonal relationships. Kornhaber et al. (2016) also highlight the concept of therapeutic listening and patient-centeredness as strategies to strengthen interpersonal therapeutic relationships. On a lighter note, it is equally productive not to use personal electronic devices in the patient care environment, as this portrays an aura of nonchalance and negligence (Caminiti et al., 2020).
It is our duty as practitioners to always rise to the occasion and well above the praxis of daily unit rounds, whenever possible, that is.
