For Week 2, you will research and obtain information about the community where you live and the health status of a target population. This information will be included in your written report and PowerPoint presentation later on.
1. Go to https://www1.nyc.gov/site/doh/data/data-publications/profiles.page and click on your community. This will give you the “Community Health Profiles” report.
Go to https://ephtracking.cdc.gov/InfoByLocation/ and type your zip code
If you live outside of New York, research using your state’s Department of Health website and other resources.
2. Identify your community and zip code in the first line of your posting. Describe the community where you live with respect to demographics, quality of life, and health data (i.e. race and ethnicity, English proficiency, social and economic conditions. housing and neighborhood conditions, maternal and child care, access to health care and health outcomes).
3. What new or interesting information did you learn about your community? Prepare a report (in paragraph form!) of your findings and post it to the Discussion Board. Look for tables and graphs that can be included in your written report and PowerPoint presentation later on. Include your reference(s) so that your peers can gain insight about your community.
