This assignment is asking you to work on Box #4 (Root Cause Analysis) on your Final A3. This can be one of the more challenging areas to work through, so we want you to apply it to the problem you are solving so we can provide you feedback early.
Directions: Submit a document using PowerPoint, Word, or one that is clearly handwritten that includes the following:
1. Your problem statement
2. At least 2 completed root causes – SHOW YOUR WORK
-You can use the fishbone diagram, driver diagram, or 5 Whys (your choice)
-Circle each root cause or somehow indicate when you have reached the root cause for each initial cause
-Remember to do the, therefore, test at the end of each root cause. This will NOT be visual for us, but you are likely to do better on this assignment if you apply that best practice.
3. Your final A3 requires 3 completed root causes, and your problem in real life may have more than that.
