2. Where you might ask do I obtain this information? From the internet. For example the California State Contractors License Board is a great source (see item 1 and 2 above). The San Bernardino County Recorders Office website is another source for the Fictitious Business Name material. (see item 4 above)
3. All of the forms must be filled out. Please type (preferred), or print clearly/legibly.
4. When searching a local contractor (#3) pick one out of the local phone directory or, use a name you know but they must be researched on the CSLB website. If there are warnings, complaints please attach the summaries not the full complaint(s) or if they are ingood standing print that out..

6. The Building permit should be from a local municipality or county in California.
