Continuum of Care
Continuum of care involves an array of programs and services that are used to help with diagnosing and treating different illnesses and diseases over time. Quality of care is important to any facility or healthcare organization. The Institute of Medicine (2018) defined quality as “the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge. Quality occurs on a continuum (unacceptable to excellent). Quality may be evaluated from the individual’s or population’s perspective.IOM. (2018).

Explore the services offered as part of continuum care as well as the quality of the HIM involved.
Include the following aspects in the discussion:
- Share (if you are comfortable) which levels of care you have used
- What was your perception of the quality of care you (or a family member) received?
- Discuss the HIM aspects involved
- Did they improve or worsen the quality of care delivered?
