I encourage you to provide more information in the Subjective and Objective portion. The subjective portion of the note should describe the presenting problem from the client’s viewpoint. It should include direct quotes from the client. I encourage you to review the previous announcement regarding goals/objectives.

Treatment planning – All clients have Goals and Interventions! Goals are where you are going (what client and yourself identify and understand) and interventions are how you are going to get there (what client and yourself identify and understand). It is goal for client to “buy in” to the goals and interventions. If safety is a risk factor – you always establish safety of your client. Client safety is paramount.

Think: do I need to refer to a general practitioner; do they need rehab or drug stabilization; do they need admission to hospital; do I need to establish a safety plan with another party? Termination – you prepare clients for this at the beginning, what is the best outcome for your client, will you refer out, can you meet all clients needs or is it best met in group therapy or other form of treatment, look at the out lining areas that may impact your clients ability to receive treatment.
