Kindly use name john instead of Frank, I changed it in some places in notes but may be missed some, so if there is written Frank, Take it as John, Dates in notes are messed up coz those are copied and pasted so kindly read according to dates and refer to case summary in power point to get some ideas about case. Please do not mention any Staff member names in power point. Please make it according to the points below and refer to ITP loop.
