General Information and Rationale
The case studies provide an opportunity for students to practice client assessment skills using objective and subjective information. The case studies also provide opportunities to practice using the baseline health assessment tool and the lifestyle medicine program tool before using them with an actual client.
Format and Requirements
The Lifestyle Medicine Case Study Workbook will be used to complete your case study’s. Detailed health information for each case are included in the Level Three chapter.
Preparation
Students should review each client case and use the associated questionnaires and logs to complete the Lifestyle Medicine Case Study Worksheet. The Lifestyle Medicine Case Study questions and worksheet can be completed electronically using the template provided. However, the case study questions and narrative will not be submitted as a part of the assignment but will be used to guide the lifestyle program development in the next step.
