A review of files shows that a company has been visited by OSHA within the past year (no citations!), but they have had 4 accidents in the last year: 1 where an 18 wheeler driver rear-ended a Greyhound while blinded by the sun in morning traffic. No injuries, but a whole lot of bad PR. Two straight truck driver accidents hitting objects while backing into loading docks, and a van driver t-boned when another driver ran a red light. 

The firm is ‘mostly’ in DOT compliance; driver files are up to date, but vehicle inspections are lacking. There is a partial written fleet management plan.
