A paper to evaluate my group member

there are 2 parts,

Part A: requires 4 pages,  has to answer the following questions. I have typed some information in the document, please follow my information to write the paper.

1) a cover sheet with your name, names of your group members, date of presentation
2) how your group developed – structure and roles
3) how and when your leader emerged and what style was used

4) was the leadership style effective – why, why not?

5) which decision making method did you generally use?

Part B: requires 2 page, the evaluation for my group member,
